PRICING AND Payment Policy
**If you will be paying at time of service (without filing through an insurance plan), please see the chart below for our time-of-service pricing.**
Dr. Spurgeon is a credentialed provider with the following insurance providers:
Blue Cross Blue Shield
Medicare Primary Plan* **
If you will be using an insurance plan, please remember you will be responsible for deductibles, copayments/coinsurances, and non-covered services.
We do not submit claims for out of network insurances. If requested, however, we can provide a superbill (detailed medical receipt) to you to submit to your insurance for possible reimbursement.
*Medicare covers a very limited number of chiropractic services. For an estimate of your out of pocket costs, reference our pricing below, and subtract $20-$50 per visit. For additional clarification feel free to reach out to our office.
**Medicare advantage plans - we are considered out of network with all carriers, and will not submit to your insurance. For amount due at time of service please reference the pricing below.

When dealing with insurance, costs can be complicated. Below is our best attempt at making it as simple to understand as possible; as it pertains to our office.
Additional Blue Cross Blue Shield information:
While we work under your insurance plans’ guidelines for services they consider “covered”, we sometimes provide, recommend, and require “non-covered” services to be completed if we are to treat you in our office. These “non-covered” services are subject to our standard fee schedule, and are due at time of service (see chart below). If we make a mistake and bill for a service that is not covered by your insurance plan, we will bill you for that service but will offer our time-of-service discount to you.
Additional Medicare information:
Medicare ONLY covers what they consider medically necessary spinal adjustments. This means all consultations, exams, X-rays, soft tissue work, laser therapy, etc are NOT covered. If your budget is tight, we can try to limit the non-covered services as much as possible. But understand that more often than not we will require a consult, exams, and possibly X-rays to be seen for care in our office.
As a Medicare patient, if you elect to follow the office recommendations for your care, understand you will be charged at time of service for non-covered services.
Depending on deductibles and coinsurances you may be billed an additional amount at a later date. This would include what Medicare considers maintenance or elective care.
